EndPoint Care Handoff

A clinic operations layer that makes every patient handoff and next action clearer.

Clinic operationsPatient readinessWorkflow orchestration

The problem

What needed to change.

Patients can leave an appointment knowing they have more to do but without the documents, instructions, or shared understanding needed to complete it. Incomplete follow-through delays care, creates repeat work, and adds avoidable pressure to already-busy clinics.

The build

What EndPoint made.

A working clinic-operations demonstration with a prioritized next-best-action queue, clinician-approved handoff builder, patient readiness search, exception ownership, and capacity insights. It is designed to sit beside an existing EMR rather than replace it.

Current proof

Built far enough
to learn from.

  1. 01Single-clerk high-flow workflow demonstrated
  2. 02Prioritized work queue and exception ownership
  3. 03Clerk and clinician approval boundaries
  4. 04EMR integration architecture mapped for future validation

Known boundary

What is not yet proven.

The demonstration does not store production patient data and is not connected to a live EMR.

The next endpoint

Add secure persistence, authentication, and notifications, then validate capacity and outcomes in a measurable clinic pilot.
